Our intensive 12 week personalised program, uniquely supports individuals at risk (whether pre-diabetes or early diabetes) or those living with Type 2 Diabetes, to give real life tools to help transform and better manage their health and life.

A medical doorway into managing diabetes

Diabetes education and clinical management is crucial for people at risk or who have been diagnosed with Type 2 Diabetes. 

The below plan of intervention integrates clinical and medical practitioners with scaled nutritional support, real time continuous glucose monitoring for an intensive 12 week program to help individuals who are at risk or living with Type 2 Diabetes have access to all aspects of education, clinical support, medical intervention, nutritional support and self management skills to prevent diabetes progression and its associated complications. 

Our unique program is focussed on making behavioural and lifestyle changes that lead to improved overall health to help better manage the condition.

Program Inclusions

DOCTOR SUPPORT

You'll check in with one of our Doctors at the beginning of the program who will provide you with a personalised medical report of your health and at the completion of the program to review just how far you've come.

DIABETES EDUCATOR SUPPORT

Your two sessions with our credentialled diabetes educator will assist and empower you to effectively self-manage the care and treatment of your pre-diabetes or diabetes.

DIETICIAN SUPPORT

Your two sessions with our accredited practising dietician will provide an individual assessment including detailed nutrition and carbohydrate assessment to help you manage and reach your diabetes targets, medications, insulin, blood glucose patterns and other factors affecting glycaemia.

DEDICATED NURSE SUPPORT

You will have regular checks with one of our registered nurses to continually review and discuss your progress and to track your success.

BODY COMPOSITION SCALES

Our unique body composition scales will be delivered to you and returned at the completion of the program. 

CALORIE CONTROLLED MEALS

You'll recieve 4 weeks of 5 days of scaled nutritional support (being breakfast, lunch, dinner and snacks (if included)) at the calorie intake recommended for you delivered from our partner Lite n Easy.

DAILY RESISTANCE TRAINING PLAN

Your daily resistance training plan is tailored to your fitness level (beginner, intermediate or advanced) and designed to help maintain your lean muscle mass to boost metabolism and improve muscular fitness. At home, or outdoors.

RECIPES + SHOPPING LISTS

Access to recipes and shopping lists for your days without the calorie controlled meals

DOCTOR'S HEALTH & WEIGHT TRACKER APP

You'll receive full access to the Doctor's Health & Weight Tracker APP to enable you to track your progress.

CONTINUOUS GLUCOSE MONITOR

You will receive a continuous glucose monitor to help you see the effects, in real time, of diet and exercise on your glucose levels for the first month of the program.

DOC BLOGS

Full access to motivational support by way of emails covering topics including healthy eating, importance of resistance training, mindful eating habits, and mastering motivation.

MEDIAL AWARD OF COMPLETION

At the end of your program, you'll be awarded a Medical Award of Completion for your efforts your incredible commitment to improving your health.

Who is this program suitable for?

This program is suitable for Phoenix Health Fund members who are 18 years or over and have been diagnosed with a chronic condition being type 2 diabetes or at high risk of developing type 2 diabetes. 

How is this program different to others?

The Doctor’s Kitchen Australia recognises that there is no one size fits all approach to diabetes management. Our personalised program, uniquely supports members living with type 2 diabetes, to give them real life tools to transform their health and life whether pre-diabetes, early diabetes or with type 2 diabetes. This is achieved through our personalised medical intervention with the guidance and support of trained general practitioners as well as experienced health professionals (credentialled diabeted educator and accredited dietician), body composition testing and real time continuous glucose monitoring to help motivate members and give them the confidence to better manage their condition, improve their physical activity levels, lose weight (and in particular reduce their body fat and visceral fat and improve their muscle), optimise their self management, and practice healthier food choices.

What are the outcomes of this program?

Our focus is in the individual's improvements in: (a) behavioural outcomes (diet, physical activity and sitting behaviours), physical outcomes (weight, body fat, body muscle, visceral fat, BMR per day, glucose levels, medication management) (b) blood pressure measures (systolic blood pressure and diastolic blood pressure), (c) perceived impact of the program on helping individuals increase their knowledge of risk factors for developing type 2 diabetes and or cardiovascular disease, (d) skills in adopting healthier lifestyle behaviours and making positive life changes that are sustainable to transform their health.  

How many meals will I receive on the 12 week Program?

You will receive scaled nutritional support during this first month of the program from our partner Lite n Easy. Following your Doctor's consultation, a dedicated Lite n Easy consultant will contact you (within 2 business days) to arrange delivery of your calorie-controlled meals at the calorie intake recommended for you. You will receive breakfast, lunch and dinner (and snacks if required) for 5 days out of 7 for the first two weeks. On the third week, you will receive lunch and dinner for 5 days out of 7. On the fourth week, you will receive dinners only. This scaled nutritional approach will help you understand the types of foods you can eat and the portion sizes to help you achieve your health and weight goals.

Can I swap out some of the meals?

As your meals are personalised to meet certain calorie requirements based on your Doctor's recommendation, it is not recommended to swap out meals from the set menus ordered by your Lite n’ Easy consultant as this will alter the calorie count. Our dedicated partner, Lite n’ Easy, have over 250+ delicious meals to make your journey a wholesome and flavoursome one! If you have special dietary requirements, i.e., vegetarian this should be discussed at your first consultation with our nurse.

What if I live outside of Lite n' Easy's deliver areas?

Lite n’ Easy currently delivery to over 90% of the Australian population with their delivery areas are expanding weekly.

What if I slip up on the program?

Remember: You Never Fail 

F.A.I.L means - First Attempt in Lifestyle-changes. 

The first step is to recognise that a slip up will happen at some stage in the process! It’s okay to slip up while you’re on the program, but the most important thing is to recognise that you have slipped up and remember why you started the program, to prevent it from happening again. Start afresh from your next meal and compensate by ensuring that you are following your daily resistance training workout plan and any additional exercise that you can fit in. If your slip ups become prolonged – we recommend reaching out to one of clinical team or speaking with the Doctor at the next consultation to discuss what options may be available to help further support you.

What if I am not losing weight?

Having a plateau in your progress during a lifestyle change is a very common occurrence – so common you expect it happen at some stage in the program. It is easy to underestimate how much to eat, so you may be eating more than you think – keeping a food diary can help identify these issues.

If you’re doing all the right things and following the Doctor's recommendation and still not losing weight, that’s what our clinical team are here for. So, at your next check in with your nurse, we recommend discussing this with them so that they can make the necessary changes to ensure that you are reaching your weight and health goals.  

Will the program help my current health condition or chronic disease?

Our Diabetes First Program is designed to give you the motivation you need to get back on track and to help you better manage this condition with guidance from our medical team. If you have other chronic diseases or conditions, we recommend that you contact us about our Health & Wellbeing program which is designed to support any underlying lifestyle-based health condition that you may have with the support and guidance of trained Doctors. If you have a lifestyle-based condition (diabetes, heart disease, cardiovascular disease, sleep apnoea, obesity, fatty liver, high blood pressure or cholesterol) our Doctors have the tools to help support your current health condition and to give you the best chance of success in transforming your health and improving your life.

Who can I speak with about the program, scales, Nurse consultations?

Who can I speak with out my meals or meal order?

Our meal partner, Lite n’ Easy, have dedicated consultants that can assist you with any queries that you may have in relation to your meal order prescribed by the Doctor, delivery, suspending or cancelling your order. For all meal related queries, please contact Lite n’ Easy on 13 15 12.

What do I do if I want to pause or stop the program?

If during the course of the program, you do not want to continue the program, the nurse will first exhaust all avenues in relation in ensuring that you are supported in the best way possible, including but not limited to, a referral to a psychologist, dietician, treating GP, or their medical specialist. Where all avenues are exhausted and you still do not want to participate, the Doctor’s Kitchen Australia will arrange for your meals and any further Doctor’s appointments to be cancelled and the health fund will be notified with reasons. You need to let us know as soon as possible by emailing us on info@thedoctorskitchen.com.au if you do not wish to continue the program.
